Annual Progress Report

All students in the Master of Psychiatric Nursing program must submit an annual progress report  to the student advisor by June 1. The progress report is designed to track student progress, and to ensure ongoing contact between the student and faculty. Registration in subsequent courses is contingent upon satisfactory progress.

The student is responsible to complete and submit parts A and B of the form to their Advisor. The Advisor will arrange a telephone meeting with the student to discuss progress and plans. The Advisor will then complete the form and make arrangements for signatures.
