Job Summary:
The Patient Care Coordinator, under the direction of the Health Service Manager, is primarily accountable for the system navigation and coordination of patient flow in the health care centre. This is achieved by the Patient Care Coordinator having an ongoing awareness and involvement in the workflow of the health care centre. Duties include bed management, discharge planning, advocating for the patient/family, identifying supply and equipment needs, assisting with staff scheduling, supporting, coaching and mentoring staff, working collaboratively with other
departments and staff on an as-needed basis, carrying out such audits as are deemed necessary to ensure that all activities are being done using Evidence Based Best Practices and Keewatin Yatthe Regional Health Authority Policies and Procedures, mission, vision, goals and values, acting on recommendations made by the Quality Performance and Improvement Committee and other Patient and Family Centred Care Committees such as the Patient Safety Committee, the Infection Control Committee and the Occupational Health and Safety Committee.

- Required Qualifications
- Bachelor Degree/Diploma in Nursing
- Basic Life Support (BLS)
- Registered or eligible for registration with the College of
- Registered Nurses of Saskatchewan (CRNS)

- Ability to make decisions, which must include coordination of staffing, and/or patient care, and/or coordination of equipment

Recruitment, Relocation and Retention Incentives may be available up to $11,500 for a return for service commitment, along with a comprehensive benefit package.
This position may also be eligible for an incentive between $30,000 - $50,000 from the Ministry of Health (subject to change).
